Absolutely slashdotted too, I see (as wget tries downloading the PDF at 300 or so bytes per second).

Recommendations
98. All trains should have windows available through which passengers can escape.

I can just see the idiots smashing them when trains are delayed at signals or are banked up.
Or idiots just smashing them while the train is on the move just for the full of it.
I thought having the EDR's on them was enough.

EDRs can only be accessed from outside the carriages. Much as I deride the Daily Telecrap, it was far more useful than other media in summarising some of the Commissioner's key points, see http://dailytelegraph.news.com.au/story.jsp?sectionid=1258&storyid=2524394
The policy of containment of passengers must be abandoned, and passenger initiated methods of escape must be provided. If there is any future accident where passengers need to escape in a hurry (eg smoke or fire) they can't rely on a train crew, particularly if the crew are dead or injured and unable to help themselves.
There is a problem with vandalism - but other systems worldwide manage to provide such escape mechanisms and effectively deal with the potential misuse of such facilities.

I can still see the idiot passengers breaking their way of the carriage when there is extensive delays like trains banking up, signal/point failures or fatalities. And I can see the idiots smashing the emergency windows out just for the hell out of it.

Haven't you seen the death tolls that have resulted from such irresponsible action by management of such places? Lock up all the life jackets on ships, so that passengers can't throw them overboard, the same sort of mentality.
Other administrations manage the problem so that a minority of idiots are discouraged from misusing emergency facilities, rather than omitting to provide them and thus subjecting the vast majority to riding in trains which are potential coffins on wheels, with no method of escape in the event of an accident, and providing difficulty and delays for rescuers in treating the injured.

Well thats stupid to have them locked up isn't it.I have seen on buses of the handle to be used to smash out the emegerency windows. If those handles are placed on trains just like they are on buses you can see whzat will happen.

Though you should see what they provide in longer distance and commuter trains in the USA - a nice big fire axe in the emergency kit, all accessible to passengers. We'd better not put those in or some CityRail staff might use them on their passengers.

Hmmm very tempting at times, there are some passengers that do deserve a good bashing.

It should also be noted that a lot of buses now carry implements to break the windows in case of emergency.
