I get plauged with spurious MS "security fixes" and other spam with large
attachments on a POP account with little genuine traffic. What I'd like to
do is:

1 Download only headers
2 Open the genuine stuff
3 Remove everything from the server that was present on the first check.

Can I do this, and if so, how?

No, which I find very annoying for the same reason. I switched to IMAP to
get around this 'problem', but it means I have a different set of
views/folders for each box.

The way my old Email client worked was that you'd open the remote mailbox
and it would display all headers. Then you'd move/copy/delete the messages
you wanted and then you'd close the mailbox. No crap or unwanted
attachments ever arrived.

In extreme cases (when the Sven worm was filling my mailbox almost as fast
as I could empty it) I've done that, but it basically it means going
through your mail twice, which I would hate. A feature from The Bat I
often used was to limit message downloads to certain maximum sizes. But I
was on dialup then.

That's what I was looking for. I can almost do it in Thunderbird, which
has an option to download hearders only, and the junk filter does a pretty
good job of leaving only the legitimate ones in the inbox. the problem is
that I'm not sure when or if the un-downloaded message bodies are removed.

For some reason unchecking "Download message bodies when checking seems to
have no effect on the same account in Opera.

I'm on dialup. For my puposes the above scheme would be fine.
Alternatively, some way to refuse (i.e. not download) attachments from
unknown senders.

Opera's POP3 access doesn't seem to support that. If you had an IMAP
account, or for newsgroups, you can download only the headers - in fact,
that is the default.

Can't distinguish between "unknown senders". There used to be a setting
for not downloading attachments (at all), but I suspect that doesn't work
with POP3 either.

Pretty much impossible with POP3, as they're not strictly "attachments"
(something that comes along with the message), but data "embedded" within
the message.

Of course, if you have a mail server that accepts the mail, then rips it
apart (message and other bits), then allows you to interact with the mail
server in the manner that you prefer, then you could simply fetch the first
text portion of a message.

IMAP is supposed to have that sort of ability, but I've yet to play with a
system that supports it.

Not yet in Opera. For my two accounts which get lots of spam, I use
mailwasher, which becomes extremely accurate at sorting the spam and
viruses over time, and has a good whitelist. You let mailwasher
automatically check your mailserver then get the mail manually in Opera
once its is processed.
It's free for a single account. The paid version handles multiple accounts
and hotmail/msn (so I only have to log in there on the odd occasion I get
a real message)

The "Security fixes" - and the "Returned message" messages for that matter
- are generally a virus. A good antivirus program would remove them for
you. I use AVG 7, by the time I see my mail the attachments have been
removed.
