As far as I know, digital cameras are ok. Or, let me rephrase that, I have not heard of them NOT being ok.

Because they have no clue, they err on the side of caution.

I am not familiar with all plane crashes, so I do not know of one where a carry on item caused a crash, if any.
However, I seem to recall seeing a documentary about a plane that crashed due to an active electronic device that was part of the cargo. The results of the investigation were new rules banning active electronic devices in the cargo. They had to be off. I don't know if this affected the passengers as well.
